The questionnaire is the set of questions reviewers answer in a performance review. Each review type has its own tab (Self review, Manager, Peer, Direct report), so self reviews and manager reviews can ask different things. You organise questions into sections and choose an answer type for each one.
Before you start
- The review must be in Draft. After launch, the questionnaire is read-only.
- Select the review types first. A questionnaire tab only appears for each selected review type.
Question types
| Answer type | What reviewers do | Notes |
|---|---|---|
| Free text | Write an open answer | — |
| Number | Enter a number | — |
| Rating | Pick a value on the review's scoring range | Can include a comment field. Only rating questions count for section weights. |
| Single choice | Pick one option | At least two options, no repeated options |
| Multiple choice | Pick one or more options | At least two options, no repeated options |
Step by step
Add sections and questions
- Go to Performance > Reviews and open the draft review.
- Scroll to Questionnaire.
- Click the tab of the review type you want to edit, for example Self review. The tab shows how many questions it has.
- Click Add section, enter a section title, and save. Sections group related questions, for example "Communication".
- In the section, click Add new question.
- Write the question, and optionally a Description.
- Choose the Answer type.
- For Single choice or Multiple choice, click Add option for each option.
- For Rating, turn on Participants can comment if you want a comment box under the rating.
- Turn on Mandatory if reviewers must answer the question.
- Click Save.
- Repeat for each review type tab.

Edit, duplicate or delete
- Open the menu (…) on a question or a section.
- Click Edit, Duplicate or Delete.
- Deleting a section deletes all the questions in it. Confirm in Delete section

Change the scoring range of rating questions
- In the Questionnaire, click Scoring range.
- Drag the range to set the lowest and highest values (between 0 and 10).
- Write a label for the values, for example "Poor" and "Exceptional".
- Click Save. The range applies to all rating questions in the review.

Preview the questionnaire
- At the top right of the draft review, click Preview.
- The Questionnaire preview shows each review type as the reviewer will see it.

Rules and behavior
- If a selected review type has no questions, then the review can't be launched ("Some of the selected review types are missing questions. You must have a minimum of one question.").
- If the review uses Final question scoring, then an Overall performance section is added automatically to the Self review and Manager tabs. You can't edit or delete it ("This section can't be edited").
- If Add potential score is on, then an Overall potential section is added to the Manager tab, with the note "Visible only to managers and performance review process owners".
- If Include competency assessment is on, then a Competencies assessment placeholder appears. The real competencies are filled in per person at launch.
- If the review uses Custom weights, then each section shows its weight, and only sections with at least one mandatory rating question can carry weight.
- The Overall performance and Overall potential questions use the company score scale from Settings > People > Reviews, not the questionnaire's Scoring range.
- If a reviewer leaves a Mandatory question empty, then they can't submit ("Complete all required fields to continue").
Limitations
- You can't change questions after the review is launched. Stop and duplicate the review if you need different questions.
- You can't add conditional (branching) questions.
- You can't import questions from a file.
- You can't reuse a single question across reviews without a template.
- You can't use different scoring ranges for different rating questions in the same review.
Troubleshooting
- Symptom: "I can't edit the questions." → Cause: the review is no longer a draft. → Fix: questions can only be changed in Draft. Duplicate the review to start a new draft.
- Symptom: "I can't give weight to a section." → Cause: the section has no mandatory rating question ("Add a mandatory rating question to add weight"). → Fix: add one, or mark an existing rating question as Mandatory.
- Symptom: "The Overall performance labels aren't the ones I want." → Cause: they come from the company score scale. → Fix: change the labels in Settings > People > Reviews > Scoring before launching. See Scores, score scales and potential.
